The Social Worker II (SWII) performs advanced social service work within a social learning matrix that requires analytical skills, consistent clinical interpretation and sound judgment based on standard principles of Social Work. Serves patients with mental, emotional and behavioral conditions and addictions. Serves as a liaison between patients and their families in obtaining assistance and rehabilitative treatment, and identifies patient needs through psychosocial assessments; treatment team collaboration, duties and meetings. Provides continuity of care functions and discharge planning with the patient, families, LMHA's and other agencies, as appropriate. Demonstrates an appropriate level of clinical expertise in discussion of case dynamics and behavior of patient that contributes to the treatment team's knowledge and understanding of the patient to provide the highest quality of care. The SW II reviews and finalizes discharge documents for LBSW/Case Managers. Supervises assigned social work student intern and provides clinical training to new staff. The SW II works under minimal supervision with extensive latitude for the use of initiative and independent judgment; and functions under the supervision of the Director of Social Services.
