The Social Worker II shift and unit will be determined to the needs of the Hospital. May require working other than normal hours, including holidays, weekends, and overtime.

The Social Worker II performs advanced social work duties under the limited supervision of the Unit Social Work Supervisor and Unit Program Supervisor with moderate latitude for the use of initiative and independent judgment. Serves as liaison between the patient, family members and outside community agencies and institutions. Works to encourage family/Continuity of Care (COC) participation in recovery and discharge planning meetings. Acts as a vital member of the psychiatric recovery team on the unit. Gathers and presents information regarding patients' environmental factors, recidivism factors, educational needs and discharge/aftercare needs. Facilitates individual contact and group education sessions with mentally ill patients. Serves as an active member of social work/unit committees as assigned. Acts as a mentor to less experienced social workers on the unit. Must understand that duties, location of job assignment and shift pattern are subject to change as determined by the administration to meet the needs of the hospital.

Must understand that the duties may include exposure to blood or other potential infectious materials.
