The Social Worker I (SWI) performs complex social service work within a social learning environment. The SW I assists in the delivery of the Social Learning/Diagnostic System, providing and receiving cross-discipline collateral supervision as directed by supervisor according to Social Learning principles. Serves as a liaison between patients and their families in obtaining assistance and rehabilitative treatment, and identifies patient needs through social assessments, treatment team collaboration, duties and meetings. Provides continuity of care functions and discharge planning with the patient, families, MHA's and other agencies, as appropriate. Provides Didactic classes as determined appropriate through recognition of special competencies. The SW I works under limited supervision with considerable latitude for the use of initiative and independent judgment and functions under the supervision of the Chief Social Worker III on the Program.

Performs other duties as assigned to ensure the efficient operation of the unit. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
