The Richmond State Supported Living Center (SSLC) is seeking a Licensed Vocational Nurse (LVN) to join an interdisciplinary team of professionals to provide clinical care to individuals of the SSLC as a (LVN II clinic nurse. In this position you will work at the SSLC campus with a dynamic group of staff and professionals who are dedicated to providing supports and services for individuals with intellectual and developmental disabilities. The Clinic Nurse performs highly complex vocational nursing work, delivering the nursing care necessary to meet the health care needs of individuals who live at the SSLC. Within the clinic setting, performs focused assessments and care intervention, such as health screenings, and collects specimens for lab work. Evaluates and documents healthcare needs and/or issues and reports health care needs and/or issues to the appropriate person(s). Responsible for the organization and overall daily functioning of the clinic. Makes clinical appointments and ensures the individuals appointment information is processed in a timely manner. Works directly with the Primary Care Provider (PCP) such as the assigned physician, nurse practitioner, consultant physician, psychiatrist, and/or dentist, and closely with other clinic and nursing staff to ensure pertinent results for their assigned clinics are entered into the database. Communicates with others, such as direct care staff, physicians/PCP, administration, and other nursing staff to ensure optimal care as part of the interdisciplinary team. Notifies the supervisor or assigned Registered Nurse Case Manager (RNCM) of trends and concerns to ensure that communication is in place to provide consistent, high quality care for all persons served. Serves as a direct care/floor nurse as needed, performing basic and complex routine and non-routine nursing tasks to promote the health of the individuals within the assigned areas and assigned shift. Instructs direct support professionals and other staff on how to provide service and care that support the individual's medical needs. Performs technical nursing procedures according to facility and state regulations. Keeps informed of and follows all agency, SSLC and unit/home policies and procedures. Provides direction and assists with on-the-job training to others such as newly hired LVNs and direct care staff. Assists Registered Nurses (RNs) with implementation of protocols, procedures and programs to improve overall health and safety of staff and individuals. Works under general supervision of a RN or other appropriate supervisor, based upon the assigned clinic, with moderate latitude for the use of initiative and independent judgment.
